5Star Life’s flagship product, The Family Protection Plan (FPP-CI) was developed to address the shortcomings of the employer paid group life insurance by providing a post-retirement benefit beyond most group life plans that end up reducing coverage amounts at age 65, when employees need the coverage most. The Family Protection Plan offers a guaranteed level premium to age 100 and maintains a level death benefit to age 100 based on current factors. FPP-CI includes a Critical Illness Benefit paid upon diagnosis of heart attack, stroke, life-threatening cancer, cardiac bypass surgery, or any terminal condition, which enhances the group health plan, by providing needed funds to pay deductable and co-payments.

The Family Protection Plan (FPP-TI) offers the same features at the FPP-CI, but without the Critical Illness feature. Both plans our offered as a money purchase plan based on weekly premiums of $4, $5, $6, $7 and $8 or in Flat Face Amounts up to $100,000. This plan is field unwritten on an “Accept/Reject” basis, with no medical underwriting requirements. Both plans are fully portable after leaving employment at the same rate. The plan is also made available to the employee’s spouse, children and grandchildren, without the employee participating.

Group Critical Illness (CI) is one of the fastest growing voluntary benefits offered to employees. Our Group Critical Illness is the most affordably priced plans on the market. We offer a Triple Benefit Feature (multi-benefit feature), up to three (3) times the face amount will be paid when diagnosed with heart attack, stroke, bypass surgery, angioplasty, cancer, organ transplant, renal failure, Alzheimer’s disease, paralysis and occupational HIV. Critical Illness insurance complements group health insurance by providing the funds to pay deductable, co-payments or to supplement a loss of income while employees recover.

Basic Group Life and AD&D – Competitive employer paid group life insurance can be quoted on a case-by-case basis. Guarantee issue limits vary by group size. Minimum group size is 5 employees.

Voluntary Group Life and AD&D can be used as an Employee Buy-up Option or quoted separately without the employer paid group life. Rates are age banded in 5 year increments. Generous Guarantee Issue limits are available and vary based on group size. A Dual Choice Option using the Family Protection Plan and Voluntary Group Life to offering the best of both plans. Giving employees a choice when it comes to voluntary life insurance is one of the richest benefits employers can offer.

Executive Level Plans can be offered any employee and their spouse when Group Life and the Family Protection simply don’t provided enough coverage to meet an individual’s specific needs. Affordable 10 and 20 Year Level Term life insurance with face amounts up to $1,000,000 is available. These plans are fully underwritten with standard underwriting requirements .
